第2回 河内オステオトミーセミナー開催報告:熱気あふれる会場と医師たちの挑戦

2026年9月12日、大阪市内の会場(TKP大阪梅田カンファレンスセンター)にて、当院の瀧上順誠医師が幹事を務める「第2回 河内オステオトミーセミナー」が開催されました。

当日は関西各地から約60名もの医師をはじめとする医療従事者が参集し、会場は大きな熱気につつまれていました。

前半は、膝の保存的治療から手術療法へのアプローチの一つである「骨切り術」に関するミニレクチャーや、実践的なハンズオン(実技指導)が行われました。

続く後半では、実際の治療に難渋した症例を持ち寄り、活発なQ&Aやディスカッションが交わされる症例検討会を実施。

医療の現場では、患者さん一人ひとりの状態に最適な治療法を提供するため、日こうした熱心な研鑽と情報共有が重ねられています。

 

「自分のひざを残す」という選択肢:人工関節と骨切り術の違いについて

ひざの痛みが進行した際、保存療法(リハビリや注射など)で効果が得られない場合の選択肢として、

主に「人工関節置換術」と「骨切り術」があります。それぞれの特徴を知ることは、ご自身やご家族の治療法を考えるうえでの大切な手がかりになります。

 

  • 人工関節置換術

痛んだ関節の表面を削り、金属やセラミックなどでできた人工の関節に置き換える手術です。

  • メリット: 痛みの軽減効果が高く、早期から安定した可動域や歩行が得られやすい点があげられます。
  • デメリット: 人工物には耐用年数があるため、将来的な再置換の可能性や、過度な高負荷運動に制限が出ることがあります。

 

  • 骨切り術(自分の骨を温存する治療)

O脚などによって偏ってしまっているひざにかかる体重のバランスを、骨を切って真っ直ぐに矯正することで、ご自身の関節をそのまま残す治療法です。

  • メリット: 自分の膝の軟骨や組織を温存できるため、術後の活動制限が少なく、スポーツや趣味、仕事への復帰がしやすい点が特徴です。
  • デメリット: 骨がしっかりとくっつくまでに一定の時間がかかることや、変形の度合いによっては適応が限られる場合があります。

On September 12, 2026, the 2nd Kawachi Osteotomy Seminar was held at the TKP Osaka Umeda Conference Center in Osaka, organized by Dr. Junsei Takigami from our clinic.

The venue was filled with great enthusiasm, gathering about 60 medical professionals—primarily doctors—from across the Kansai region.

  • The First Half: Featured mini-lectures and hands-on practical training focusing on “osteotomy,” an effective surgical approach that bridges conservative treatment and joint replacement.

  • The Second Half: Included a case-study session where participants brought in challenging cases from actual clinical practice, leading to lively Q&A and active discussions.

In the medical field, such dedicated study and information sharing are continuously carried out to provide the most optimal treatment for each individual patient.

“Keeping Your Own Knee”: Understanding the Difference Between Total Joint Replacement and Osteotomy

When knee pain progresses and conservative treatments (such as rehabilitation or injections) no longer provide sufficient relief, the main surgical options are Total Joint Replacement and Osteotomy. Understanding the characteristics of each is an important first step in considering treatment options for yourself or your family.

  • Total Joint Replacement (Arthroplasty)

    • What it is: The damaged surfaces of the joint are removed and replaced with artificial joints made of metal or ceramics.

    • Pros: High pain-relief effectiveness; stable range of motion and walking ability are often achieved relatively early.

    • Cons: Artificial parts have a limited lifespan, meaning potential future revision surgery and restrictions on high-impact sports.

  • Osteotomy (Preserving Your Own Bone)

    • What it is: The weight-bearing axis of the knee—often thrown out of balance by conditions like bowlegs (O-legs)—is corrected by cutting and straightening the bone, leaving your natural joint completely intact.

    • Pros: Because your natural cartilage and tissues are preserved, there are fewer post-operative activity restrictions, making it easier to return to sports, hobbies, and work.

    • Cons: It takes a certain amount of time for the bone to fully heal, and applicability may be limited depending on the degree of deformity.
